Session Summary
CMS is on a fast-track path to reducing utilization and readmissions by highlighting Social Determinants of Health (SDoH). CMS has already begun updating the MDS to reflect questions that should give providers insight into a beneficiary's needs outside of the facility. In this session we will review CMS 's strategy and how providers can utilize managed care benefits to conquer SDoH.Presenters
