This session will discuss the impacts of high rehospitalization rates in skilled nursing centers. There will be a discussion about how to target high-risk residents and action steps to reduce rehospitalizations, along with tangible steps to make a difference in your rehospitalization rates.

About the Presenter
Amina Dubuisson has been a nurse for over 24 years. She earned an Associate of Science degree in nursing from Miami Dade College and become a Registered Nurse. She received her Masters of Science in Nursing, Business Administration and Healthcare Management from the University of Phoenix; and her Doctorate in Nursing Practice from Chamberlain University. She is currently the Vice-President of Clinical Services at Ventura Services of Florida, Inc. She is the Vice Co-Chair of the Senior Clinicians Council from the Florida Health Care Association. She received the Florida Health Care Association Rising Star in Nursing Award and was recognized as an emerging nurse leader who has demonstrated excellence in nursing and person-centered care practices. She was recognized by Greystone Healthcare Management for her volunteer works in the community. Amina's biggest achievement has been her ability to give back to the community through volunteering and providing care to the underserved population through various Medical Missions and Health Fairs. Every year, she travels to Haiti at least twice in Medical Missions. She believes that everyone has a purpose in life and her purpose is to help people.
