Session Summary
Root Cause Analysis (RCA) is a method of problem solving used for identifying the root causes of faults or problems. It is widely used in IT operations, telecommunications, industrial process control, accident analysis, in medicine and the health care profession. This session will provide participants with a better understanbding of a root cause analysis in relation to the issues and challenges that are dealt with in the long term care environment. With increased scrutiny of required Reportable and Adverse Incidents, having a more thorough understanding and being able to drill down and identify root causes will assist the nursing home administrator with being in compliance with the updated CMS Requirements for Participation.

About the Presenter
Dr. Kendra Ferrero is a multi-state licensed RN, board certified in Gerontology She also holds an active Nursing Home Administrator license in several states. She earned her MBA from Warner University and her Doctorate degree with a specialty in Health Care Administration from Northcentral University. She has worked in the post-acute environment since 1977. She has been involved in the delivery of educational presentations in skilled nursing since 1984. She was part of the original RUG11 Demonstration Project when it was implemented in NY in 1985. She has held positions as a Director of Nursing, Regional Nurse, VP of Clinical, Nursing Home Administrator and Regional Vice President of Operations. Dr. Ferrero currently works with Citadel Care Centers as the Vice President of Operations and Clinical Management. She has sat on numerous FHCA committees for the past 10 years. She is a Lead Senior Examiner for AHCA/NCAL Quality Program. She is a certified Risk Manager and has a passion for the skilled nursing environment, ensuring quality care is provided to the residents/patients.
Peggy Norris

About the Presenter
Peggy Norris has been a Registered Nurse in the State of Florida since 1972 and has 50 years of health care experience. She is also a Certified Dementia Practitioner as of November 2016, SMQT certified with CMS/AHCA 2003 as Surveyor and Quality of Care Monitor and has been employed since 2012 with Signature HealthCARE, LLC as Signature Care Consultant for the Signature HealthCARE skilled nursing centers in the Panhandle. She is currently an active member of the FHCA Senior Clinicians Committee and Risk and Compliance Council.
