Session Summary
To prevent, detect and mitigate harm related to medications, long term post-acute care providers must have a robust medication review program. How does your organization perform medication reviews? What education is provided to staff administering medications? What processes are in place related to review of medications upon admission? The answers to these questions may prevent harm to residents in your center. In this session, speakers will review strategies to prevent, detect and mitigate harm related to medications.
