Session 21 | Hospital Readmissions: Harmful for Residents and Costly for Centers

Session Summary

Starting in 2018, skilled nursing centers have been subject to a penalty of up to 2% of their Medicare reimbursement for higher-than-average rates of hospital readmissions and public posting of their rankings. There are opportunities to improve resident-centered care and protect skilled nursing centers financial viability by preventing unnecessary and unwanted acute care transfers. This session will gather best practices for reducing skilled nursing center acute care transfers and hospital readmissions while promoting resident and family satisfaction through an action-oriented sharing model among session participants.
