This session provides a practical, operator-focused look at where the value-based landscape is headed and what it will mean as the industry transitions toward 2027. Participants will gain a clear understanding of today's primary value-based pathways, including the Medicare Shared Savings Program (MSSP), ACO REACH (High Needs), and Institutional Special Needs Plans (I-SNPs)- and how these models increasingly influence referral patterns, length of stay, clinical expectations, preferred provider relationships, and overall financial performance for skilled nursing providers. The session will also explore how emerging and evolving CMS initiatives, such as the new CMMI model LEAD, may further shape the next phase of value-based care.

A respected leader in healthcare transformation, Keith Persinger brings nearly 30 years of executive experience driving operational, clinical, and financial strategy across acute and post-acute settings. As CEO of Provider Partners Connect Care, Keith leads one of the nation's few value-based care organizations dedicated to long-term care, partnering with skilled nursing and assisted living facilities to succeed in High-Needs ACO REACH and other advanced care models. Prior to this role, Keith served as Chief Performance Improvement Officer for the University of Maryland Medical System and held executive positions including CFO and COO at the University of Maryland Medical Center. His work has consistently focused on delivering sustainable outcomes through collaborative, data-informed approaches. Keith is a frequent presenter at national conferences on value-based care and long-term care innovation. He holds both a bachelor?s degree and MBA from Loyola University Maryland.
