Session Summary
This presentation will review the background and purpose of Transitions of Care (TOC) and discuss the differences in how meds are managed in the inpatient and outpatient settings. This session will explore the most common medication discrepancies seen during care transitions, as well as review some best practices to consider for building support around care transitions with your interdisciplinary team. This session will cover the common barriers to care transitions, particularly reviewing that key transition from hospital to center and the opportunities to develop TOC programs.Presenters

About the Presenter
Alan Obringer is the president and owner of Guardian Pharmacy of Orlando. Alan is licensed as a pharmacist and consultant pharmacist in Florida and holds a faculty position with the University of Florida, College of Pharmacy as an Assistant Clinical Professor, since the fall of 2002. He became a Certified Geriatric Pharmacist in 2012 and serves as a preceptor for pharmacy students, offering both a Geriatric and Long-term Care practice site. Alan earned his pharmacy degree from the University of Rhode Island and he is a strong advocate for the profession of pharmacy in long-term care and is very involved in the changes that affect our industry.Michael Samarkos
